▸ REQUIREMENTS CHECKLIST

• FOCCUS / REFOCCUS Marriage Preparation

• Natural Family Planning (NFP) Program (Diocese Approved)

• Declaration of Freedom to Marry (Witness Form II)

• Baptism Certificate with marginal notations (annotations)

• First Communion Certificate

• Confirmation Certificate

• Form V – Final Marriage Preparation Form

▸ MARRIAGE PREPARATION​​​​

Couples must complete a diocesan-approved marriage preparation program, including FOCCUS / REFOCCUS and parish formation.

▸ NATURAL FAMILY PLANNING (NFP)​​​​

An approved diocesan NFP program is required. Instructions and registration details are provided by the parish office.

▸ SACRAMENTAL RECORDS​​

All certificates must be current and valid.
Please notify the parish office if:

• One party is not Catholic

• Any sacrament is missing

▸ FINAL STEPS​​

Once all requirements are completed:

• Contact the parish office

• Provide both names of the couple

• Schedule a meeting with the priest

• Confirm available wedding dates

📌 Wedding dates are not reserved until all requirements are completed and approved.​​

▸ DEPOSIT

A $200 deposit is required after priest approval to reserve the wedding date.
This deposit is applied toward the total wedding fee.
